What Is Occlusal Reduction?

Occlusal reduction is the controlled removal of tooth structure from the biting surface to create space for the restoration, so that it has the right fit, material thickness and occlusal alignment. The challenges are a uniform depth, avoiding over-reduction and preserving tooth structure. Freehand reduction often ends in uneven clearance, extra chair time, or a restoration that does not fit well.

Which Depth: 1.5 mm or 2.0 mm?

Choose the depth from the minimum thickness that the restorative material requires, which differs for zirconia, lithium disilicate, metal-ceramic and gold. Follow the restoration manufacturer's guidelines, add any extra clearance your case needs, and pick the bur that matches.

How to Do Occlusal Reduction with a Depth Cutting Bur

  1. Plan the clearance for the material you will use.
  2. Select the bur: AD15 for 1.5 mm or AD20 for 2.0 mm.
  3. Cut depth channels along the occlusal anatomy, following the central groove and the cusp inclines, with water spray.
  4. Reduce the tooth between the channels to their depth, following the cusp inclines so that you keep the original anatomy instead of leaving a flat table.
  5. Check the clearance with a bite registration or a silicone index, in closure and in excursions.
  6. Refine and smooth the surface with a finer bur.

Depth-Controlled Bur vs Wheel and Barrel Burs

Wheel and barrel burs are freehand reduction burs: you judge the depth by eye, which is fast but depends on experience. A depth cutting bur sets the depth for you, so the reduction is more even and repeatable, which helps most when you do not prepare crowns every day. Many clinicians combine them: a depth cutting bur to set the depth and a wheel or barrel bur for broad reduction.

  • Replace the bur when the diamond is worn, because a worn bur cuts slower and generates more heat.
